About the Trial

The Systolic Blood Pressure Intervention Trial randomized 9,361 adults over 50 with hypertension and cardiovascular risk to systolic targets under 120 versus under 140. The intensive arm cut major cardiovascular events by 25 percent and all cause mortality by 27 percent. The trial stopped early after 3.26 years. Benefits came at the cost of more hypotension, electrolyte abnormalities, and acute kidney injury, mostly manageable. SPRINT challenged the view that higher systolic targets were adequate for older adults. It ignited global debate about blood pressure targets, measurement methods, and frailty, and drove US guidelines to redefine hypertension at 130 over 80.
