About the Trial
The Action to Control Cardiovascular Risk in Diabetes trial tested intensive glucose control targeting HbA1c under 6.0 percent in over 10,000 patients with established type 2 diabetes and high cardiovascular risk. The intensive arm was stopped 17 months early after 22 percent more deaths: 257 versus 203 in the standard group. Hypoglycemia and rapid lowering after long standing poor control were implicated. The trial redefined safety limits, showing that lower is not always better. Guidelines subsequently moved toward individualized, often less stringent HbA1c targets for older and high risk patients.
