Key FindingTight glucose and blood pressure control in type 2 diabetes reduces complications

About the Trial

The UK Prospective Diabetes Study followed over 5,000 newly diagnosed type 2 diabetes patients for a median of 10 years. Intensive treatment with sulfonylureas or insulin lowered HbA1c to 7.0 percent and reduced microvascular endpoints by 25 percent. Metformin in overweight patients reduced myocardial infarction and death. The blood pressure arm showed tight control at 144 over 82 reduced strokes, heart failure, and diabetes related death. Unlike type 1 diabetes, glucose had modest macrovascular effects. The 10 year post trial follow up showed legacy effects of glucose control persisting for a decade after the trial ended, while the blood pressure benefits faded once control was relaxed.
