About the Trial
The Diabetes Control and Complications Trial randomized 1,441 type 1 diabetes patients to intensive versus conventional insulin therapy. Intensive control, targeting near normal HbA1c, reduced retinopathy onset by 76 percent, slowed progression by 54 percent, and reduced nephropathy and neuropathy. The trial proved the glycemic hypothesis: high glucose causes microvascular complications. It transformed type 1 diabetes care, leading to wide adoption of multiple daily injections or insulin pumps and HbA1c monitoring. Observational follow up in EDIC found the former intensive group had 42 percent fewer cardiovascular events, a long term effect described as metabolic memory.
