About the Trial

The Scandinavian Prostate Cancer Group randomized 695 men, mostly Swedish, with newly diagnosed localized prostate cancer to radical prostatectomy or watchful waiting. After 29 years of follow up, surgery reduced prostate cancer death by 11.7 percentage points, a relative risk of about 0.55, and also reduced all cause mortality, with the largest gains in men under 65. The trial proved treatment of localized prostate cancer can save lives, countering nihilism from the PSA era overtreatment debate. It informed the contrasting trials PIVOT and PROTECT, which showed many screen detected cancers need no immediate treatment. Together they defined modern risk stratified prostate cancer care.
