The Randomized Evaluation of Long Term Anticoagulation Therapy trial randomized 18,113 atrial fibrillation patients to blinded doses of dabigatran or open label warfarin. Dabigatran 150 mg twice daily reduced stroke or systemic embolism by 34 percent versus warfarin with similar major bleeding, and 110 mg matched efficacy with less bleeding. After 50 years of warfarin, requiring frequent monitoring and dietary caution, a fixed dose oral alternative became available. RE-LY launched the direct oral anticoagulant class and triggered a wave of non inferiority trials: ROCKET AF, ARISTOTLE, and ENGAGE AF. Anticoagulation for atrial fibrillation expanded dramatically worldwide.
