About the Trial
The Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands randomized 500 patients with anterior circulation proximal occlusions to intraarterial treatment plus usual care or usual care alone. Thrombectomy doubled the chance of independent recovery, modified Rankin 0 to 2, from 19 to 33 percent. Earlier device trials, IMS III, SYNTHESIS, MR RESCUE, had been negative and dampened enthusiasm. MR CLEAN, run in a small country with organized stroke care, reversed the field. ESCAPE, EXTEND IA, SWIFT PRIME, and REVASCAT soon confirmed it, as did the 2016 HERMES pooled analysis, and thrombectomy became the most effective acute stroke intervention ever, now standard within 24 hours in selected patients.
