Key FindingMetoprolol reduces death in heart failure, reversing beta blocker dogma

About the Trial

The Metoprolol CR XL Randomized Intervention Trial in Congestive Heart Failure randomized 3,991 patients with moderate heart failure to controlled release metoprolol or placebo on top of standard therapy. The trial stopped early: metoprolol reduced all cause mortality by 34 percent and sudden death by 41 percent. For decades beta blockers were considered forbidden in heart failure because they acutely depress contractility. MERIT-HF, with CIBIS II and COPERNICUS forming the trio, proved chronic beta blockade protects the failing heart from adrenergic toxicity. Beta blockers moved from contraindication to core therapy, one of the great reversals in cardiology.
