About the Trial

The Modification of Diet in Renal Disease trial randomized 840 patients with chronic kidney disease to usual versus low protein diet and moderate versus strict blood pressure control. The primary analyses found no significant benefit of protein restriction or strict pressure targets on GFR decline overall, though secondary and long term analyses suggested benefit in proteinuric patients. The trial was among the first major CKD outcomes studies and its blood pressure data fed into later meta analyses showing that lower targets help proteinuric kidney disease. MDRD also popularized the MDRD equation for estimating GFR, which standardized CKD staging worldwide, arguably its most durable contribution.
