About the Trial

The Magpie Trial randomized 10,141 women with pre eclampsia to magnesium sulfate or placebo. Magnesium more than halved eclampsia, 0.8 versus 1.9 percent, and likely reduced maternal death. Children followed to 18 months showed no harm. Collaborative Eclampsia trials had already established magnesium as the treatment for eclampsia over diazepam and phenytoin. Magpie extended prevention to pre eclampsia, making magnesium standard for severe pre eclampsia worldwide. The drug is inexpensive and the trial, run across 33 countries, exemplifies how obstetric emergencies yield to pragmatic randomized evidence. The later BEAM trial found antenatal magnesium reduced cerebral palsy in preterm infants.
