About the Trial
LOOK AHEAD randomized over 5,100 overweight or obese adults with type 2 diabetes to intensive lifestyle intervention targeting 7 percent weight loss or diabetes support and education. The intervention group lost 8.6 percent body weight, improved fitness, HbA1c, and quality of life, and reduced sleep apnea and urinary incontinence. Despite these gains, the trial was stopped for futility after 9.6 years: cardiovascular events were not significantly reduced. The result was humbling, showing weight loss alone may not fix established vascular risk. Secondary benefits were substantial, and the trial refined realistic expectations for lifestyle interventions in long standing diabetes.
