About the Trial

The Pre Exposure Prophylaxis Initiative trial randomized 2,499 men and transgender women who have sex with men to daily oral tenofovir emtricitabine or placebo. PrEP reduced HIV acquisition by 44 percent overall, but by up to 92 percent among participants with detectable drug levels, establishing that adherence drives protection. iPrEx led to the 2012 FDA approval of Truvada as PrEP, the first biomedical HIV prevention pill. Follow up trials, Partners PrEP and TDF2 in heterosexuals, confirmed class wide benefit. PrEP transformed HIV prevention into a consumer controlled daily option and became a cornerstone of ending the epidemic strategies.
