About the Trial

HIV Prevention Trials Network 052 randomized 1,763 HIV discordant couples to immediate versus delayed antiretroviral therapy for the infected partner. Immediate treatment reduced linked HIV transmission by 96 percent, effectively to zero when viral load was suppressed, alongside reducing disease progression in the treated partner. Led by Myron Cohen, the trial established treatment as prevention. The result contributed to global guidelines recommending treatment for everyone diagnosed, drove UNAIDS 90 90 90 goals, and underpins U equals U, undetectable equals untransmittable, dismantling stigma and criminalization arguments. PARTNER and PARTNER2 studies later confirmed zero transmissions across tens of thousands of condomless sex acts when suppressed.
