The Future Revascularization Evaluation in Patients with Diabetes Mellitus trial randomized 1,900 diabetic patients with multivessel disease to CABG or drug eluting stents. Surgery reduced the primary composite of death, heart attack, or stroke at 5 years, 18.7 versus 26.6 percent, driven by lower death and heart attack, at the cost of more stroke. The benefit consolidated over time. FREEDOM combined with SYNTAX to establish that diabetic multivessel disease generally favors surgery. Diabetes became a decisive factor in revascularization strategy, and heart teams routinely weigh it when choosing between PCI and CABG.
