About the Trial
Dapagliflozin and Prevention of Adverse Outcomes in Heart Failure randomized 4,744 patients with reduced ejection fraction to the SGLT2 inhibitor dapagliflozin or placebo. The drug reduced the primary composite of worsening heart failure or cardiovascular death by 26 percent, with benefit whether or not patients had diabetes. Following EMPA-REG cardiologists suspected a heart failure mechanism, and DAPA-HF proved it directly. The benefit emerged within weeks. SGLT2 inhibitors became one of four pillars of heart failure therapy, alongside ARNI, beta blocker, and MRA. DELIVER extended the benefit to preserved ejection fraction, covering the full heart failure spectrum.
