About the Trial

The Clinical Antipsychotic Trials of Intervention Effectiveness, funded by NIMH rather than industry, randomized 1,493 schizophrenia patients to olanzapine, quetiapine, risperidone, ziprasidone, or perphenazine, an old first generation drug. Olanzapine was slightly more effective but caused weight gain and metabolic problems. Perphenazine matched the newer drugs for most patients, undermining the premium pricing of second generation antipsychotics. Discontinuation rates were high for all drugs, 74 percent within 18 months, revealing chronic management realities. CATIE reshaped prescribing toward metabolic monitoring and cost conscious selection and stands as a landmark publicly funded pragmatic psychiatric trial.
