About the Trial

The Anti TNF Trial in Rheumatoid Arthritis with Concomitant Therapy randomized patients with active rheumatoid arthritis despite methotrexate to infliximab infusions plus methotrexate or methotrexate alone. Combination therapy reduced signs and symptoms substantially more than methotrexate alone and, uniquely for the time, halted erosive joint damage on X rays. TNF blockade, proven shortly before by Maini and Feldmann groups, validated cytokine targeted therapy in autoimmune disease. Infliximab, approved by 1999 shortly after etanercept, helped open the biologics era: adalimumab and eventually multiple classes targeting TNF, IL 6, B cells, JAK pathways. Remission became a realistic treat to target goal for many patients.
