About the Trial
The ARDS Network trial randomized 861 patients with acute respiratory distress syndrome to traditional 12 mL per kg tidal volumes versus lung protective 6 mL with plateau pressure under 30. The low volume arm reduced mortality from 40 to 31 percent, an absolute 9 percent gain, and increased ventilator free days. Overdistending injured lungs perpetuates damage, the volutrauma concept, proven decisively. Lung protective ventilation was widely adopted within years, and the trial is often cited as one of the most influential in intensive care. The ventilator management lessons echoed loudly during COVID 19 ventilation strategies.
